DO YOU NEED TO REPORT A WORKPLACE ACCIDENT/INJURY?

Clover Park School District is subject to Washington industrial insurance laws and has been approved by the state to cover its own workers’ compensation benefits. Self-insured employers must provide all benefits required by law. The Department of Labor & Industries regulates employers’ compliance with the law. If an employee becomes injured on the job or develop an occupational disease, he/she will be entitled to workers’ compensation benefits.

EMPLOYEE’S RESPONSIBILITY:

Employee notifies supervisor immediately of the event/accident. Employee completes an Employee Accident Report. In the event employee will be seeking medical treatment, employee must call the PSWCT Nurse Triage Helpline at 1-833-928-6877 as soon as employee is able.

PURPOSE:

Reporting is a state requirement and employee must complete before the state can assist with medical needs.

  1. Employee Accident Report. Employee completes within one business day of the event, and signs only the employee section. Employees should make a copy for own records. If employee is unable to complete the form at the time of accident, personnel at respective location can complete and submit on behalf of the employee.   2. Submit the completed Employee Accident Report to employee’s supervisor for completion. This form will then be forwarded to the District Risk Management office.
  3. If employee is not released to return to work, it is employee’s responsibility to update supervisor and Human Resources by providing copies of all doctor notes (Activity Prescription Forms), submitting absence slips, entering absence in Frontline (formerly Aesop). It is also important to notify HR with weekly progress updates.

SUPERVISOR’S RESPONSIBLITY: Notify and document.

  1. PURPOSE: Required reporting and accident prevention. Immediately notify the District Risk Management office when anyone reports an injury or accident.
  2. Employee Accident Report. After an employee completes the top section of the form, the supervisor completes and signs the supervisor's section (lower portion of form).
  3. Submit the completed Employee Accident Report to the Risk Management office within two business days of the employee’s workplace injury.

PHYSICIAN'S RESPONSIBLITY:

NOTE: Clover Park School District is a self-insured employer under Washington State law. The physician must use the self-insured accident report and not complete state L&I paperwork.

The medical provider will then submit all required paperwork directly to Puget Sound Workers’ Compensation Trust.

DOES TIME MISSED FROM WORK DUE TO AN INJURY COUNT AGAINST EMPLOYEE’S LEAVE?

If employee misses work due to an injury and doctor certifies employee is unable to work, Puget Sound Workers’ Compensation Trust may pay for a portion of the lost wages called "time‑loss compensation."

However, the first 3 days in a row of missed work, immediately following the injury, are considered a waiting period. These days count against an employee’s sick leave. After 3 days, employee will be required to complete a Time Loss Election Form (provided by HR) to indicate payment provisions.

HOW DO THE TIME-LOSS COMPENSATION BENEFITS WORK?

Time-loss compensation benefits will not cover all an employee’s lost wages, only a fraction. Plus, this requires an employee’s medical provider's ongoing certification. If employee receives time-loss compensation benefits from Puget Sound Workers’ Compensation Trust, any compensation received through the District would be adjusted to reflect payment from Puget Sound Workers’ Compensation. In other words, in the event the employee receives workers’ compensation benefits, the District will pay employee the difference between workers’ compensation benefits and regular pay (if employee has paid leave).
